Therapist – Outpatient Mental Health Clinic (OMHC)
Services Description
Responsible for overall Therapist coordination and management of all OMHC related activities.
Specific Duties/Responsibilities
- Assist with coordination of intakes, diagnostic evaluations, transfers, and discharge meetings
- Writing contact notes per visit with a member, Individual Treatment Plan (ITP), monthly summaries and discharge summaries; and completing OMS Surveys
- Coordinate Individual Treatment Plan (ITP) meetings as deemed necessary by Program Director
- Collaborate with other Treatment Team members (PRP, Nurse, Medical Director, and Program Director)
- Contact appropriate agencies and other parties in suspected cases of abuse and neglect and develop written reports
- Conduct individual, group, and family therapy/counseling as needed;
- Maintain contact with caregivers, case managers, attorneys, etc
- Facilitate therapeutic groups with clients to assist in the development of social and interpersonal skills
- Ensure overall health, safety and well being of clients;
- Support participants and their families by informing them of available community services
- Refer participants, as necessary, to agencies providing community services
- Assist in the delivery of other required program services, including meeting the psychosocial needs of participants
- Other designated responsibilities.
Assessment and Evaluation
- Review each client’s ITP to determine goals and objectives that need to be carried out in the OMHC
- Perform Diagnostic Evaluations as needed, initial
- Write progress notes per session, monthly progress reports, and initial and updated ITP more if needed, for each client; and
- Prepare reports for intake, transfer, or discharge meetings
- Observe staff and client interaction
Training
- Confer at least one hour per month with therapeutic and rehabilitative staff members to develop or improve therapeutic programming or to provide group therapy education classes for participants
- Conduct in-service training sessions for staff as designated by the director
- Participate in any company recommended training
Skills/Qualifications:
Provide evidence of a Master’s Degree in Social Work, Psychology, and Counseling from an accredited college or university.
Valid licensure in the state of Maryland to include: LGSW, LCSW-C, LICSW, LCPC, LPC within the State of Maryland and the District of Columbia
Have at least one year of experience or training working with clients with pediatric, adults, and elderly with or without developmental disabilities whose condition, due to mental and physical health needs and characteristics, require intensive services
Be at least 21 years of age
Obtain certification in cardiopulmonary resuscitation (CPR) and renew the certification annually
Complete first aid training in addition to all other training required by COMAR
Be able to communicate with clients
Be able to recognize emergencies and be knowledgeable about emergency procedures
Be able to read and write the English language
Must be a US citizen or alien who is lawfully authorized to work in the US; and
Satisfactory physical examination by a licensed practitioner including, but not limited to freedom from communicable disease as confirmed by an annual PPD skin test/chest x-ray or documentation from a physician stating that the person is free from communicable disease
